In the event any fees are paid to counsel for an estate relieved or during an accounting period, there shall be attached to the account the appropriate computation form, to wit:

Computation Form for Attorney Fees - Estate Relieved, Appendix B2
Computation Form for Attorney Fees - Guardianships and Trusts, Appendix B3
