The application filed on behalf of a minor shall be captioned, e.g., “In re: John Doe, a minor,” and the application of each minor shall be assigned a separate case number.
The application shall set forth the following, if applicable:
- (c) That either or both of the parents are deceased, or their whereabouts are unknown to the applicant; and
- (d) That the parents are divorced or separated, and the name and address of the individual to whom custody has been awarded.
Include, if applicable, an attachment executed by a parent who is not the applicant, and by a minor, is he is at least fourteen (14) years of age, consenting to the granting of the application.
Have attached thereto a current statement of a physician attending the minor describing the injuries sustained, the extent of recovery there from, and a prognosis of the condition of the minor.
