All Lorain County Vendors are required to complete the Vendor Affidavit form as required by HB 694.

• HB694 Vendor Affidavit Fill-In Form - enables you to download the form, complete it in your Adobe Acrobat Reader, print it and mail it to the Auditor's Office. This form requires Adobe Acrobat 7.
• HB694 Vendor Affidavit Print Only Form - must be printed, then completed and mailed to the Auditor's Office.
