Real Estate Forms

Conveyance and exempt forms are available in printable or fillable form on the website. You only need to submit one form with your transfer. Just a reminder, we do not accept handwritten forms. All forms must be typed and filled out completely. All required information for the form can be found on the auditor's website.

Forms that are incomplete and/or have incorrect fees will not be processed and will be returned to you. We will not hold any type of document. You may call our office if you have any questions before sending in your transfer. Thank you in advance for your compliance.

Pursuant to Ohio Revised Code § 4705.01, beginning August 1, 2011 the Real Estate Office will no longer fill out Conveyance or Exempt forms.  All documents being presented for transfer must have the proper Conveyance or Exempt form submitted.

What to bring with you for a Tax Release (ET12 & ET14):

Copy of the death certificate
Copy of the accounts on bank letterhead/ or the bankbook
Copy of the will or appointment papers from probate court
Driver's license or proof of I.D.
2 copies of estate form #12 (front and back)
1 copy of estate form #14 for each account over $25,000
