Please complete this registration form in its entirety and include it with your deposit and remit to address at bottom of form. You will also need to complete the Medical History & Waiver form prior to first day of training.  

      

2008 - SUMMER SESSION

Must have a Medical History form and Waiver to Train (May pick up 1st day)

    All Sessions will be starting June 1st 2008 and Every Sunday to August 1st.  (Off June 29th & July 27th) 

at Elyria South Recreation Facility

  2) ______      4:00 pm. - 6:00 pm. (7th - 9th grade)  OVERALL TRAINING (Covers all Skills)

  3)  ______      6:30 pm. - 8:00 pm. (9th - 12th grade) HITTING/BLOCKING -  ALL HITTING SKILLS 

                                                             $130.00 Specialized Hitting (Includes a Mandatory training shirt)

  4) ______      8:00 pm. - 9:30 pm. (8th - 12th grade) SETTING & Serving - ALL SETTING SKILLS            

  5) ______      8:00 pm. - 9:30 pm. (8th - 12th grade) DS/LIBERO & Serving - Defense & PASSING SKILLS
